How can Physical Therapy help TMJ (Temporal Mandibular Disorder)? - Monday, October 25, 2010

Our Physical Therapists use manual therapy combined with functional exercises to rehabilitate TMJ dysfunction. The osteopathic model provides us with a framework to evaluate the patient systematically. This information is combined with a functional evaluation of the musculoskeletal system to determine the areas of greatest restriction or limitation.

When evaluating for temporomandibular dysfunction, we typically question the patient about any parafunctional habits that may be overloading the joint complex, such as teeth grinding, fingernail biting, and/or a diet heavy in hard, crunchy, or chewy foods.

Working in concert with a patient¹s dentist or oral surgeon, an acrylic appliance may need to be fabricated to address nighttime bruxism. Early condylar translation with mouth opening can also contribute to complaints of painful joint noises. Neuromuscular reeducation is frequently employed to improve TMJ ³tracking.² When limited opening or restricted motion is identified, gentle intraoral mobilizations are commonly indicated.

Assessment of the cervical spine and craniosacral system is included to maximize the patient¹s response to manual physical therapy and improve overall outcomes.

Treatment emphasis is geared toward decreasing pain, normalizing range of motion, and restoring function. Finally, we put a priority on teaching patients to participate actively in their treatment regimen by performing home exercise to prevent recurrence.
