Patient Discharge Survey
All patients who have been recently discharged from their physical therapy program at Orthopedic & Spine Therapy are eligible to take our 10 question online survey, powered by Survey Monkey.
Each patient who completes the survey is eligible to win a $25 gas or restaurant gift card upon completion of the survey, and the submission of their current email address. All winners will be drawn monthly, and will be verified with their patient records to confirm that they were an actual patient.
